The hyperpigmentation of the ectopic ACTH syndrome is due to the pigmentary action of the very high levels of ACTH, although occasionally an additional melanocyte-stimulating hormone (MSH) derived from its ACTH precursor molecule (pro-opiocortin) is produced.
